Disabled Children's Service
The Disabled Children's Team is a specialist social work service based in Hackney Ark, children and young people centre for development and disability. It works with children and their families where the risks and needs meet a Tier 3 Threshold as outlined in the Hackney Partnership Child Wellbeing Model. This includes child protection assessment and casework and services for looked after children, Carer's Assessments and Transition Assessments for post 16s.
For those families that are eligible for Tier 3 provision, they can have a care support package through a named provider or by Direct Payments. They can also be referred to other services for family support and early help, for example, play schemes, leisure activities and voluntary organisations.
In addition, the Short Break scheme is administered in conjunction with the Disabled Children Service in which Hackney's children and young people in receipt of Disability Living Allowance (DLA) at medium or high rate are able to access their local short break core offer. Transition Services (age 14-18) are also provided in collaboration with the Disabled Children Service and all young people in receipt of services will have an assessment at age 16 as a referral mechanism to assess for eligibility for Adult Social Care Services.
In addition, the Transition Targeted Outreach Team work with young people who are expected not to be eligible to access Adult Social Care services, for example, have moderate learning disabilities, school action plus. They provide an 'offer of support' that includes health and well being, education and training for employment through a person centered framework targeted at the pace of the young person to achieve their goals.
