How to get social care support
Health and Wellbeing Services can arrange support to help you with daily living tasks and remain as independent as possible. You will need to contact our Information and Assessment Service on 020 8356 6262 to tell us about your needs.
A staff member will talk with you about what you are able to do and look at ways to help you overcome any difficulties. The information you give us will be treated confidentially and we will only share the information with people who need to know about your situation to help you. We will always ask you permission before sharing information about you.
Who is involved in the assessment?
Often a home visit will be necessary to assess your needs. The person (the assessor) who comes to talk to you will be trained to give you information and advice about the choices available to you. They will carry identification including a photograph, and will always tell you their name and how you can contact them.
If a relative or friend provides lot of care for you, they have the right to ask for carers 'needs assessment'. During the assessment you can have a friend or relative with you to help you explain your needs and put forward your views. We can also put you in touch with a voluntary organisation who can provide someone to help you in this way.
What happens after the assessment?
You will receive a copy of your Outcome Focussed Support Plan. If your carer has also undergone a assessment of their needs, they will also receive of copy of their own plan. We can translate your plan into your preferred language or format.
What happens if I disagree with the assessment?
If you do not agree with the conclusions of the assessment, you can appeal the decision. You have the right to complain if you are dissatisfied with an assessment decision or the way in which the assessment has been carried out. Initial contact for needs assessment is through a variety of routes depending on the person's age and requirements.
