Help leaving hospital

 

The main function of the Hospital Social Work Team is to make assessments of need under the NHS and Community Care Act 1990 and to plan appropriate discharge care services in conjunction with ward and community-based staff at Homerton University Hospital.

You can ask to see a social worker during your stay in hospital. The nurse looking after you will arrange this if you ask.

If you were receiving help before you went into hospital the social worker will arrange for this to be started again for your return home. If you were not receiving help or need more help than before, the nurse will contact the Hospital Social Work Service to arrange for an assessment.

Help that may be available to you, from the Council, could include:

It may be that when you leave hospital, you will need to go into a residential or nursing home. This could be temporary, to give you time to recover more fully, or to get services started for you at home, or it might be permanent. Our staff can help arrange this, or give you advice if you are able to make your own arrangements.

 

Service providers and partners

 

The providers of this service are First Response Duty Team, and Hospital Social Work Team.

 

Referrals and accessing the service

 

This service normally requires referrals from hospital staff and professionals, while staying or receiving treatment in hospital.  Help and advice via either the Hospital Social Work Team or Access Team. If necessary a home visit will take place or social workers will liaise with meals service and occupational therapy to ensure that appropriate services are in place upon discharge.
