Learning Disabilities
The Learning Disability Service is a joint service between the Teaching Primary Care Trust and the Council's Community Services Directorate. The service provides the following areas of service for people who have learning difficulties -
The Access Service
Is the first point of contact for referrals. Each referral begins a process of assessment, carried out jointly by Health and Care Management staff, which determines each person's needs.

The Mainstream Service
Including the Care Management functions of -
- Progression - Supported Living and Housing
- Review - carried out by Health and Social Care Assessors
and the Specialist Health Team who provide a range of specialist Health services to improve the health and well being of service users, including -
- Counselling
- Occupational Therapy
- Physiotherapy
- Community Nursing
- Psychology
- Psychiatry
- Speech and Language Therapy
- Behaviour Therapists
Day Opportunities
Day Services Projects provide a range of activities for people with learning disabilities. The service is arranged as six Project Teams, each with a specialist focus -
- Homes and Gardens
- Performing Arts
- SportsAbility
- Me and My Life
- Arts and Crafts
- Food and Drink
The teams provide and develop opportunities for people to access activities within community settings in line with their assessment needs.
The Community Resource Service
The Community Resource Service offers individualised and focused support to assist service users to develop links within the mainstream community and take on valued roles.
