Lee House Referral Procedure
Lee House provides a Rehabilitation and Employment service to people with enduring mental health issues and referrals will be accepted for membership to Lee House from the following agents:
- The four locality Mental Health Teams within the London Borough of Hackney
- The John Howard Centre and CASS
- The Rehabilitation Unit based at Homerton Hospital
- C.O.S.T. Team (Community Outreach Support Team)
- The Assertive Outreach Service
We will accept referrals from other agencies for example. Community Psychiatric Occupational Therapist, Community Psychiatric Rehabilitation Team, and We will be happy to meet the person who is referring you for an informal visit, but for the referral to go any further, they must be directed through one of the above agencies.
To consider clients for Lee House membership, we require the following documentation to support the referral:
- Lee House Referral form
- Current Care Program Approach (within the last 6 months, 12 months for Standard Level) / Needs Assessment
- Current Risk assessment communication form (within the last 6 months, 12 months for Standard Level)
In addition, all referrals to Lee House will be assessed on an individual basis and merit, in relation to how they meet our criteria.
Lee House are not able to accept anyone for membership who is unsuitable or, who is considered to present a risk to our members and staff’s safety.
