Adult social care in Hackney is changing
Future services will give you more choice and control over the care and support you get. The Council, NHS, and local organisations are working together to make sure that everyone is listened to and helped. Better information and advice will help you get the services that you want.
If you are eligible for ongoing support you will have the option of planning it yourself. You could choose to have money, called a personal budget, to pay for it. We are aiming to offer this new service from mid-2010.
How the system will work
You contact the council. We discuss your needs with you to find out if you're eligible for ongoing support.
- If you're not eligible we will put you in touch with local organisations, other council services, or the NHS to get you the right support that will help improve the quality of your life.
- If you are eligible, we will give you a short course of support. This may involve rehabilitation or therapy for example. After this we will review your progress to see if your needs have changed. It may be that you don't need ongoing support anymore.
- If you do need ongoing support we will re-assess you.
- This will give us an idea of your budget so you can choose what services you want. It will also tell us if you have any care charges to pay. The services you choose must be based on what you think will improve your life.
- You then put together your support plan. You can have help from us, your friends or family to do this.
- You agree your plan with us.
- Now you can get on with your life. We will review your progress regularly, and adjustments will be made to your plan and budget if necessary.
